"Refractory ventricular tachycardia, also known as “electrical storm,” is defined as at least two or more separate ventricular tachycardia (VT) or fibrillation (VF) episodes requiring intervention in a 24-hour period.
So what to do?
- ABC
- Search for causes
- Start with ACLS
- Beta-blockers
- Consider procedures to reduce the sympathetic drive
- Consider increasing the amount of energy given during defibrillation
- Special consideration for patients with Brugada syndrome
- Consider sedation
- Consider cardiac catheterization post-ROSC"
