In the acutely ill patient without an apparent source, consider musculoskeletal, cardiac, nervous system, and abdominal sources of sepsis. A thorough physical examination, abdominal CT scan, and lumbar puncture may help reveal the source of sepsis. Keep the differential broad when treating the hyperthermic, tachycardic patient, as many conditions can mimic sepsis. A review of the patient’s medical history and medications may be invaluable in pinpointing the source of the patient’s fever and tachycardia.
